Abstract : Introduction Surgery still represents the treatment of choice of esophageal carcinoma. In recent years both resectability of the tumor and operative mortality rates (10) have been improving due to the experience gained in this type of surgery. Radiation therapy and chemotherapy should still be considered as adjunct to surgery: when used preoperatively, they may decrease the mass of the tumor and increase its resectability. The following items will be discussed: preoperative staging, indications to surgery, operative approach, extent of resection, anastomotic techniques, postoperative complications, long term results, the role of adjuvant therapies, and prognostic factors.
